A.ativan contains lorazepam which has CNS depressant property so it reduces the respiratory drive in response to hypercapnia(increased CO2).Theophylline is a drug commonly used in asthmatics in whom bronchoconstriction can lead to reduced ventilation leading to retention of CO2.CNS responds to it by hyperventilation(incresed rate and effort of breathing).to prevent CNS depression thephylline is not used with ativan.

 I am already on symbicort and ventolin nibulizer as well as montulukast singular tablets for my sever asthma, and i am being told by a Specialist that i could try Theophylline, but after looking into im wondering if it might be more beneficial for me to take additional formoterol.? also what are the diff re ce in how the two drugs work?
A.In terms of first choice I would have suggested the folmeterol first, it has much fewer side effects than theophyllines. Though once upon a time I was a fan, ( many years ago!) theophyllines have a very narrow therapeutic window and, even in patients who tolerate them for a while often there are late side effects such as nausea, muscle cramps and abdominal pain. Ideally also blood levels should be checked periodically to ensure that they do not become toxic. Folmeterol has a large range of dose related response, for the 12 microgram unit the daily dose for regular use should not exceed 4 inhalations, however occasionally up to a maximum of 6 inhalations may be allowed within a 24-hour period.

 My data (from class) shows that theo has higher Kd values than caffeine, but theo has shorter retention times than caffeine in an HPLC analysis. The Kd tells me theo is more polar but the retention times are throwing me off.....Any help?? serious answers please :) Thanks.
A.Theophylline is 1,3-dimethyl-7H-purine- 2,6-dione and Caffeine is 1,3,7-trimethyl-1H-purine-2,6 (3H,7H)-dione. Look carefully at these structures and look at the difference in what is being dissociated for the Kd values. As for the HPLC, you did not specify if you were using a normal phase or reverse phase (C-18) column. Was the mobile phase only water, methanol and/or acetonitrile or does it contain acetic acid? In a typical C-18 reverse phase HPLC analysis, the non-polar compounds adsorb into the column longer than the more polar compounds (which remain in the mobile phase longer and so elute first).

A.This is highly unlikely due to the amount of theophylline in the cup of tea. Even the fact that theophylline is much more stronger than caffeine, the 1mg which is in the cup of tea (only black tea) is not enough to stimulate.
